Just fill out a member application form and provide a photocopy of your birth certificate, driver’s license or passport.
The application also asks you to name a beneficiary for your account. In the event that you die before you begin receiving retirement payments from SPP, the funds in your account will be paid to the beneficiary, less withholding tax.
